This issue of Surgical Pathology Clinics is devoted to Soft Tissue Tumors, the first in this series was presented in 2011. This issue addresses the most difficult diagnostic challenges and focuses on differential diagnosis in soft tissue tumors. Each presentation is accompanied by abundant histologic slides to display the diagnostic differences. Additionally, authors selected two to five diagnoses they find can be particularly difficult, with an emphasis on how to approach such lesions on biopsy samples where relevant and the role of ancillary studies. Topics include coverage of diagnostically challenging: Vascular lesions; Retroperitoneal “Fatty” tumors of adults; Smooth muscle neoplasms; Chondro-osseous lesions of soft tissue; Pediatric tumors; Epithelioid tumors; Spindle cell neoplasms of the retroperitoneum; and Peripheral nerve sheath tumors. Also presented are: Non-mesenchymal mimics of benign and malignant soft tissue tumors; Soft tissue tumors with overlapping molecular findings; Recently characterized soft tissue tumors; Benign mimics of sarcoma; Advances in molecular methods in the analysis of soft tissue tumors and therapeutic implications; and Myoepithelial tumors: an update. Leona Doyle and Karen Fritchie lead this issue of experts in soft tissue pathology.

Intensive care units (ICUs) provide comprehensive, advanced care to patients with serious or life-threatening conditions and consequently, a significant amount of end-of-life care (EOLC). Indeed, approximately 20% of deaths in the U.S. are associated with an ICU stay, and nearly half of U.S. patients who die in hospitals experience an ICU stay during the last 3 days of life. Despite the commonality of the ICU experience, ICU patients typically suffer from a range of distressing symptoms such as pain, fatigue, anxiety, and dyspnea, causing families significant distress on their behalf. Thus, there is a growing imperative for better provision of palliative care (PC) in the ICU, which may prevent and relieve suffering for patients with life threatening illnesses. Effective palliative care is accomplished through aggressive symptom management, communication about the patient and family’s physical, psychosocial and spiritual concerns, and aligning treatments with each patient’s goals, values, and preferences. PC is also patient-centered and uses a multidisciplinary, team-based approach that can be provided in conjunction with other life-sustaining treatments, or as a primary treatment approach. Failure to align treatment goals with individual and family preferences can create distress for patients, families, and providers. If implemented appropriately, palliative care may significantly reduce the health care costs associated with intensive hospital care, and help patients avoid the common, non-person centered treatment that is wasteful, distressing, and potentially harmful. Due to the success of many PC programs, administrators, providers, and accrediting bodies are beginning to understand that palliative care in the ICU is vital to optimal patient outcomes.

Evidence base in 2015 remains a subject of controversy for surgeons related to its application in surgery that cannot be approached as it is in medical evidence. Academic surgeons acknowledge that evidence base is necessary and private practitioners know it is woven into the fabric of their practice. Dr. Lisa Ishii and Dr. Travis Tollefson, editors of this publication, are at the forefront of clinical use of and research into evidence based surgery. The Oxford Centre system of evidence is used for this issue. Currently, evidence is dominant in the reconstructive aspect, moreso than the cosmetic aspect; as such, this resource focuses on the nerve and microvascular procedures. Topics include Facial vascular anomalies, Cleft lip and palate; Trauma; Facial reanimation; System reviews and metanalyses; and Skin care, Laser treatments; and Rhinoplasty. Audience for this resource is facial plastic surgeons, otolaryngologists, plastic surgeons, laser therapists, dermatologists, and skin researchers.

Public Health Mini-Guides: Diabetes provides up-to-date, evidence-based information in a convenient pocket-sized format. Diabetes is a worldwide public health concern and is being referred to as the ‘global epidemic of diabetes’, the ‘silent epidemic’ and the ‘diabetes timebomb’. The increasing incidence of diabetes, the heavy burden of morbidity and mortality associated with diabetes, and its spiralling healthcare costs, underpin the importance of a public health approach to its prevention and management.
