Dans le cadre d'une pathologie dégénérative lombaire, l'arthrodèse lombaire ou lombosacrée le plus souvent instrumentée est largement utilisée, soit en complément d'une chirurgie de décompression, soit de façon isolée dans le cadre de la chirurgie de la lombalgie. Une ostéosynthèse lui est le plus souvent associée, son but étant de rigidifier les segments intervertébraux et d'accroître le taux de fusion. Cependant, ces arthrodèses instrumentées et rigides ont montré leurs limites (risque d'accélération de l'usure des segments intervertébraux) et ne sont parfois pas adaptées, surtout chez les jeunes sujets. Cet ouvrage fait le point sur les avancées techniques dans les alternatives à l'arthrodèse instrumentée rigide classique : ostéosynthèse souple ou par abord réduit, prothèses discales partielles ou totales, mais aussi l'application des cultures cellulaires à la régénérescence discale, qui devient ainsi la technique biologique la plus récente sur le plan clinique. Les auteurs rappellent par ailleurs l'importance de l'évaluation clinique et paraclinique pour un résultat durable du traitement. Ils font également le point sur la biomécanique et l'étude de l'équilibre sagittal permettant d'éclairer les phénomènes anatomopathologiques d'usure des disques sus-jacents.
