Le « sixième sens », c'est la perception de soi-même dans le champ gravitationnel terrestre. Sa pathologie, c'est le vertige qui impacte tout l'organisme : la vision, l'équilibre, la vie neuro-végétative et même le psychisme. L'oublié d'Aristote se niche au coeur de l'oreille interne, dans le labyrinthe vestibulaire, aux côtés de l'audition. Mais il reste sous contrôle du cervelet. Grâce à l'imagerie, les tableaux cliniques répertoriés au fil des décennies se sont dédramatisés. Cependant, la plupart des diagnostics restent fondés sur la clinique : VPPB, vertiges migraineux, déficits vestibulaires brusques et Menière. L'acuité de l'examen vestibulaire s'est décuplée par la vidéonystagmoscopie et la vidéonystagmographie. Chacun peut voir, enregistrer et revoir des nystagmus que seuls quelques « prophètes » avaient pu observer au siècle dernier. Il en ressort une logique impeccable reposant sur un répertoire de six mouvements oculaires générés par chacun des six canaux semi-circulaires. Les combinaisons de nystagmus périphériques doivent être ensuite distinguées des désinhibitions centrales. Composé de trois grandes parties, cet ouvrage est très illustré et enrichi de plus de 40 vidéos en ligne, de protocoles d'examen et de nystagmus pathologiques. La première partie consacrée aux aspects anatomophysiologiques permet de mieux comprendre les nystagmus et mouvements anormaux observés dans la pathologie de l'oreille interne et de l'appareil vestibulaire. La deuxième partie consacrée à la clinique a été augmentée d'arbres diagnostiques et de vidéonystagmographies en ligne. Deux nouveaux chapitres sont proposés, l'un consacré au VPPB du canal horizontal et l'autre aux VPPB rares ou complexes. La troisième partie consacrée à la réhabilitation, avec notamment un catalogue d'exercices vestibulaires, s'est attachée dans son dernier chapitre à préciser l'importance du bilan vestibulaire dans les chutes à répétition des sujets âgés. Cet ouvrage s'adresse non seulement aux ORL, mais aussi aux kinésithérapeutes, aux médecins de rééducation ainsi qu'aux neurologues et aux gériatres.
