  • Liver Transplantation, An Issue of Clinics in Liver Disease

    • 1st Edition
    • Volume 21-2
    • Roberto J. Firpi
    • English
Cirrhosis from hepatitis C (HCV) is now the most common indication for liver transplant (LT) in the U.S., but between 2004 and 2013, new LT listings for NASH increased by 170%. Unfortunately, fibrosis progression leading to cirrhosis, liver cancer, and liver decompensation continues to occur after transplantation. Once cirrhosis and decompensation are evident, patient survival is poor and repeat LT is considered to improve outcomes. Therefore, the never-ending thirst for new approaches in the management of patients pre- and post-transplant has led to a very promising future in transplantation, thought there is much to learn to achieve better patient outcomes. This issue of Clinics in Liver Disease addresses the core areas to achieve better patient outcomes, with articles devoted to coagulopathy before liver transplant, challenges in renal failure before LT, LT for acute alcoholic hepatitis, LT in the pregnant patient, bariatric surgery and LT,and MELD Scores in prioritization of LT, to name a few. Readers will place a high value on the current state of liver transplantation in this issue.

    • Lauren B. Gerson
    • English
Endoscopic access to the small bowel has advanced significantly since the introduction of video capsule endoscopy and deep enteroscopy in early 2000. Other major advances have occurred in imaging modalities involving computed tomography and magnetic resonance studies. Due to these advances, the recent 2015 ACG guideline changed the terminology from “obscure” to “small bowel” bleeding because the majority of cases now can be found to have a small bowel source. The improvements in technology have advanced our ability to visualized vascular findings, inflammatory lesions, and small bowel neoplasms. Articles in this issue are devoted to these improvements in technology.

  • Obesity and Gastroenterology, An Issue of Gastroenterology Clinics of North America

    • 1st Edition
    • Volume 45-4
    • Octavia Pickett-Blakely + 1 more
    • English
In the last three decades, the prevalence of obesity has doubled with over 600 million obese adults worldwide. The gastrointestinal complications of obesity are well publicized; however, the role of the gastrointestinal tract in the development and treatment of obesity is often understated. From the pathophysiologic role of gut hormones and the microbiota, to the purposeful, anatomic derangement of the gastrointestinal tract that is utilized as a treatment for obesity, it is imperative that gastroenterologists understand the full scope of obesity in relation to the gastrointestinal system. Studies suggest that in the U.S. obesity is underrecognized and undertreated by health care providers. Given its complex sociology, pathophysiology, and treatment, obesity, like many other diseases, requires a multidisciplinary approach. Therefore, gastroenterologists must be equipped with the relevant knowledge of obesity that will empower them to properly manage their patients.

  • Endoscopy in Inflammatory Bowel Disease, An Issue of Gastrointestinal Endoscopy Clinics of North America

    • 1st Edition
    • Volume 26-4
    • Maria T. Abreu
    • English
The field of inflammatory bowel disease has evolved in many ways. The first has been the revolution in medical therapy for the disease. With the advent of biologic therapy, the aspirational goal of therapy has become mucosal healing. These medications have also made it possible to avoid or delay surgery for ulcerative colitis and Crohn's disease. Although non-invasive strategies such as MR or CT enterography or stool-based assays are helpful, they have not been able to replace the prognostic information provided by endoscopy. Simultaneously, the quality and spectrum of tools available to perform endoscopy has widened. High resolution endoscopes are now the norm and are nearly as good as chromoendoscopy for identifying dysplasia in ulcerative colitis patients. Most gastroenterologists also perform dilations but do not generally do this for patients with IBD. Finally, newer devices such as enteroscopes, endoscopic ultrasound, and endomicroscopes are being incorporated into the algorithms to diagnose and treat complications of IBD. This issue highlights all of these advances by the experts in their fields.

    • 2nd Edition
    • Jean-Philippe Dillenseger + 2 more
  • Alcoholic Liver Disease, An Issue of Clinics in Liver Disease

    • 1st Edition
    • Volume 20-3
    • Stanley Cohen
    • English
Alcohol abuse remains a significant problem world-wide. The most commonly affected organ remains the liver with a risk of alcoholic liver disease (ALD) which can range from asymptomatic to alcoholic hepatitis to alcoholic cirrhosis. In 2010, alcohol-attributable cirrhosis was responsible for 0.9% of all global deaths. Given the sheer magnitude of alcohol abuse and alcoholic liver disease, an update on this topic is pertinent and relevant. This issue will focus on a variety of topics including alcoholism, the pathogenesis of ALD, the spectrum of ALD, the pathology of ALD, and the long-term management of patients with ALD. Alcoholic hepatitis is a particular type of ALD with a high mortality. This issue will explore the syndrome of alcoholic hepatitis, its prognostic markers, and the available therapies. Because alcohol intake can influence so many other conditions of the liver (especially hepatitis C), articles in this issue will also explore the impact of alcohol abuse on other liver conditions. In addition, several other topics such as nutritional therapy for ALD, ALD and liver cancer, and ALD and infection risk will be explored. Also, even though it is quite controversial, we will also explore liver transplant as a therapy for alcoholic liver disease including alcoholic hepatitis.
